¡Hola a todos en la comunidad OCD! Espero que se encuentren muy bien, el dia de ayer me encontraba en una guardia en mis pasantias rurales que fue un tanto tranquila aun asi, llegaron al menos 50 pacientes los cuales fueron valorados junto a mi equipo de guardia y salieron con sus tratamientos para las patologias que presentaban en ese momento, luego de varias horas dentro del ambulatorio, conseguimos ir a dormir a eso de las 02:00 AM hasta las 06:00 AM que empezaron a llegar nuevamente pacientes.

I hope you are doing well, yesterday I was on duty in my rural internship which was a bit quiet even so, at least 50 patients arrived who were evaluated with my team on duty and left with their treatments for the pathologies they presented at that time, after several hours in the clinic, we managed to go to sleep at about 02:00 AM until 06:00 AM patients began to arrive again.
Uno de los primeros pacientes fue un pediatrico de 7 años quien acudio al ambulatorio luego de haberse caido de su propia altura en donde se golpeo en la cabeza y presento una pequeña laceración que amerito 3 puntos de sutura.
One of the first patients was a 7-year-old pediatrician who came to the outpatient clinic after falling from his own height where he hit his head and had a small laceration that required 3 stitches.

Pasadas las horas, hubo un momento de calma mas o menos entre las 02:00PM hasta las 04:00PM por lo que decidi dar una pequeña vuelta al ambulatorio por si conseguia algo que hacer, durante la espera en el cuarto de consulta recibí una visita no muy esperada y se trata de Freya, una pequeña gatita que fue rescatada por los médicos rurales que hacen vida en el ambulatorio, dicha gata se esterilizó de manera que pudiese vivir en el cuarto de médicos sin el miedo de que en algún momento esta llegase a quedar embarazada y tener que lidiar con más gatitos, actualmente es la mascota del ambulatorio y todo el personal le tiene un gran cariño.
After a few hours, there was a moment of calm more or less between 02:00PM and 04:00PM, so I decided to go around the clinic to see if I could get something to do: 00PM so I decided to take a little walk around the clinic in case I got something to do, during the wait in the consultation room I received a not very expected visit and it was Freya, a small kitten that was rescued by the rural doctors who make life in the clinic, this cat was sterilized so that she could live in the medical room without the fear that at some point she could become pregnant and have to deal with more kittens, she is currently the mascot of the clinic and all the staff has a great affection for her.
En horas de la noche nos llega otro paciente pediátrico esta vez una niña de 8 años de edad quien se encontraba jugando cuando en un mal movimiento que realizó sufrió un esguince en el tobillo derecho, una de las cosas que más me sorprendo de esta niña fue la fortaleza y capacidad que tenía para soportar el dolor, es importante recalcar que este tipo de lesiones son muy dolorosas debido al movimiento de las estructuras anatómicas y por lo general los nervios se ven afectados por presión ocasionando una molestia increíble, pero esta niña soportaba los movimientos que debemos realizar para asegurarnos de volver a su sitio el tobillo.
In the evening hours we received another pediatric patient, this time an 8 year old girl who was playing when in a bad movement she suffered a sprained right ankle, one of the things that surprised me the most about this girl was the strength and ability she had to withstand the pain, It is important to emphasize that this type of injuries are very painful due to the movement of the anatomical structures and usually the nerves are affected by pressure causing an incredible discomfort, but this girl supported the movements that we must make to ensure that the ankle returns to its place.
A la hora de realizar el examen físico en estos pacientes es importante observar el miembro contralateral o el miembro sano para encontrar si existe algún cambio significativo entre una y otra, en el caso de la niña se podía observar un aumento de volumen en la región del maleolo derecho por lo que debíamos inmovilizar este miembro y referir a un traumatólogo para que le realizara una rayos X en la zona afectada y decidiera si se debía colocar una inmovilización de tipo yeso o no.
When performing the physical examination in these patients it is important to observe the contralateral limb or the healthy limb to find if there is any significant change between one and the other, in the case of the girl we could observe an increase in volume in the region of the right malleolus so we should immobilize this limb and refer to a traumatologist to perform an X-ray in the affected area and decide whether to place a cast type immobilization or not.

En el ambulatorio no se contaba con guata ni con yeso por lo que teníamos que improvisar para hacer la férula así que usamos cartón de base y restos de campo estériles para fijar el miembro e inmovilizarlo, nuestra meta es evitar que el sitio de lesión tenga algún tipo de movimiento pues esto puede ocasionar que haya más daño, cómo bien aprendí durante mi pasantía clínica en el hospital central de San Cristóbal en el servicio de traumatología, tenemos que llevar el miembro a su posición anatómica y allí fijarlo, para no lesionar mas el tobillo, tuve que realizar el modelo de la ferula en el otro miembro de manera que la niña no pasara por dolor a medida que iba calculando el tamaño del carton en dicho miembro. Luego de aproximadamente una hora de trabajo concluimos con nuestro deber y los familiares de la niña la llevaron inmediatamente a un centro especializado.
In the outpatient clinic there was no wadding or plaster so we had to improvise to make the splint so we used cardboard base and sterile field remnants to fix the limb and immobilize it, our goal is to prevent the injury site from having any kind of movement because this can cause more damage, as I learned during my clinical internship at the central hospital of San Cristobal in the traumatology service, we have to take the limb to its anatomical position and fix it there, in order not to injure the ankle more, I had to make the model of the splint in the other limb so that the girl did not go through pain as I was calculating the size of the cardboard in that limb. After about an hour of work we concluded our duty and the girl's relatives took her immediately to a specialized center.
Las horas en las guardias pueden llegar a pasar muy lento algunas veces, otras como en esta ocasión avanzan de manera veloz por el hecho de siempre estar ocupados sacando la consulta y ayudando a todos los pacientes que se acercan, mi grupo el día de ayer fue bastante eficiente y conseguimos dormir más de 3 horas algo que muy pocas veces sucede.
The hours on call can sometimes go by very slowly, others, like this time, go by very quickly due to the fact that we are always busy taking out the office and helping all the patients that come in, my group yesterday was quite efficient and we managed to sleep for more than 3 hours, something that rarely happens.
